WE GLADLY ACCEPT ALL INSURANCE*
As a service to our patients, our practice will file any insurance program including non-managed care, indemnity (traditional) and PPO out-of-network as a courtesy. We are not part of any managed care networks. Our accounting staff will prepare all of the necessary forms for your dental benefits if you provide your insurance card. However, we remind you that your specific policy is an agreement between you and your insurance company. All co-payments and deductibles are due at the time of service. In the event your dental insurance determines a service to be "not covered" or results in less coverage than anticipated, you will be responsible for payment. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefit coverage will be.
* We are not accepting Medicaid at this time.

We do NOT file secondary insurance.

Charges not paid by your insurance company within 90 days will be payable by you. A service charge of 18% per annum accrues on any portion of a balance remaining over 90 days.

The responsibility for payment of services rendered to dependent children whose parents are divorced rests with the parent seeking treatment. Any court ordered responsibility judgment must be determined between the individuals and cannot be considered by this office.  

* If you have the BCBS Federal Insurance plan or BCBS of NC Insurance plan, the payment for services rendered will be mailed directly to the patient. Therefore, you must pay up front at the time of service.

* A 24 hours notice is required for all appointment cancellations or changes. For hygiene appointments, the fee is $25.00 per patient. The fee for a broken appointment with the doctor is $50.00 per hour.

* If you are 15 minutes late for your appointment, you may be rescheduled for another day. This will be considered a broken appointment.

Financing

Care Credit (interest- free payment plans available)

Care Credit is a healthcare line of credit for treatments and procedures for your entire family. It is a credit card designed for your health and beauty needs. You can receive NO INTEREST financing by making your minimum monthly payments and paying the full amount due by the end of the promotional period. Interest will be charged to your account from the purchase date if the promotional purchase is not paid in full within the promotional period. Minimum monthly payments are required.
