Cancellation & Late Policy
All appointment cancellations or changes require at least one business day's notice. For any appointments cancelled or altered within this time, we reserve the right to charge a broken appointment fee of $50.00 per hour.
Please note: If you are fifteen minutes late for your appointment, you may be rescheduled for another day. This will be considered a broken appointment.

Insurance
As a service to our patients, we accept all Insurances and will file any primary insurance as a courtesy. It is important to note we are only In-Network with Delta Dental.
Our accounting staff will prepare all of the necessary forms for your dental benefits if you provide your insurance card. However, we remind you that your specific policy is an agreement between you and your insurance company. All co-payments and deductibles are due at the time of service. In the event your dental insurance determines a service to be "not covered" or results in less coverage than anticipated, you will be responsible for payment. Charges not paid by your insurance company within ninety days will be payable by you. A service charge of 18% per annum accrues on any portion of a balance remaining over ninety days. We do not file secondary insurance for you, but you are welcome to file independently.
Our staff will gladly request a pre-treatment estimate from your insurance company so that you will know what your benefit coverage will be.
The responsibility for payment of services rendered to dependent children whose parents are divorced rests with the parent seeking treatment. Any court ordered responsibility judgment must be determined between the individuals and cannot be considered by this office.
*Liberty Dental is Out-of-Network and will not pay for services rendered at our office.
*We are not currently able to accept Medicaid.

You can receive NO INTEREST financing by making your minimum monthly payments and paying the full amount due by the end of the promotional period.
